Hi James, yes this will work for your EZGO cart.
This would not let you add three to both sides. You would not be able to mount the extra buckle and strap.
My cart is a Club Car Precedent Proffesional, and this kit did not fit. You have to secure the anchor bar so that both front & back belts can be attached to it, & the Professional model is not designed for this kit.
You can cable one member of each belt across the back of the battery box & to the frame for an anchor.
Hopefully Yamaha is better suited.

Yes, in Florida I"ve had two carts DOT passed and approved with no issues.
I put it on a 2011 ez go. There was plenty of adjustment in the frame for the canopy support bars
